ABSTRACT

Objective:To investigate the impact of the interaction between triglyceride-glucose(TyG)and obesity-related anthropometric indices on hypertension risk. Methods:A total of 3 356 permanent residents over 40 years were recruited from Songnan community, Baoshan district, Shanghai. All participants underwent questionnaire surveys, physical examinations, and biochemical parameter tests. TyG-WHR, TyG-WHtR, and TyG-BMI were obtained by multiplying TyG index with waist-to-hip ratio(WHR), waist-to-height ratio(WHtR), and body mass index(BMI), respectively. Logistic regression models were used to investigate the associations of the three indices and hypertension. The interactions of TyG index and obesity-related anthropometric indices on the risk of hypertension were analyzed in different genders. Results:After multivariable adjustment, the fourth quartile of TyG, TyG-WHR, TyG-WHtR, and TyG-BMI demostrated higher risk of hypertension compared with the first quantile, with OR(95% CI)2.93(2.37-3.62), 3.22(2.59-4.01), 4.33(3.46-5.42), and 4.07(3.27-5.06). Two abdominal obesity-related indicators WHR(RERI=1.52, 95% CI 0.12-2.92; AP=0.32, 95% CI 0.06-0.59)and WHtR(RERI=2.41, 95% CI 0.64-4.19; AP=0.39, 95% CI 0.15-0.63; SI=1.88, 95% CI 1.10-3.21)had a biological interaction with TyG index on the risk of hypertension in female, but not in male. Conclusion:The combination of TyG and obesity-related indicators is significantly correlated with the increased risk of hypertension, which may be influenced by gender and fat distribution type.

ABSTRACT

Objective: To determine the diagnostic accuracy of pneumococcal antigen detection indiagnosis of pneumococcal meningitis in children. Methods: Purulent meningitis wasdiagnosed according to European Society for Clinical Microbiology and Infectious Diseases(ESCMID) guideline between July 2014 and June 2016. Along with a cerebrospinal fluid(CSF) culture, pneumococcal antigen detection in cerebrospinal fluid (CSF) was performed, and further identification of pathogens was done with 16S rDNA-PCR and high-throughputsequencing. Results: CSF samples collected from 184 children (median age of 1.92 mo).CSF culture was used as the gold standard. 46 (25%) had positive results for culture and 10(5.4%) were pneumococci; 34 (18.5%) were pneumococcal antigen positive. The sensitivityand specificity of pneumococcal antigen detection were 100% (95% CI: 89.4%–100%) and86.2% (95% CI: 96.4%–99.9%), respectively. 92.3% (12/13) were confirmed by nucleic aciddetection to be pneumococci. Conclusions: Pneumococcal antigen detection in CSF hasadequate sensitivity and specificity in diagnosing pneumococcal meningitis.

ABSTRACT

Objective:To investigate the clinical features and treatment effect of children with central nervous system demyelinating diseases and seropositivity to myelin-oligodendrocyte glycoprotein (MOG) antibody. Methods:The clinical characteristics of 28 had seropositivity to MOG among 115 children with central nervous system demyelinating diseases and who were hospitalized at Department of Neurology, Children′s Hospital of Zhejiang University School of Medicine from March 2017 to February 2019 were retrospectively analyzed.Results:Twenty-eight patients were included in this study, including 10 males and 18 females, with the ratio of male/female of 1.00∶1.80, and the median age of 7 years and 9 months. The clinical manifestations were diverse, including encephalopathy symptoms such as hea-dache, vomiting, and drowsiness (13/28 cases), vision loss (7/28 cases), spinal symptoms (6/28 cases), cerebellar symptoms such as ataxia, slurred speech (4/28 cases), convulsions (2/28 cases), and cranial nerve symptoms (1/28 cases). Among 24 cases who underwent CSF detection, 10 patients (41.7%) had slightly increased white blood cells, 2 patients (8.3%) had elevated protein, 6 patients (25.0%) had positive MOG antibody, and CSF-restricted oligoclonal band was negative in all 24 patients.Twenty-five cases (89.3%) showed brain magnetic resonance imaging (MRI) abnormalities, including cerebral white matter (20/28 cases), cerebellum (10/28 cases), cerebral gray matter (9/28 cases), thalamus/basal ganglia (6/28 cases), brainstem (6/28 cases), optic nerve (5/28 cases), and corpus callosum (4/28 cases). Of the 28 cases, 13 patients had spinal cord involvement, involving cervical spinal cord in 10 cases, thoracic cord in 9 cases and lumbar spinal cord in 5 cases; besides, 8 cases of them had long segmental spinal cord lesions with ≥ 3 segments.Fourteen patients received the visual evoked potentials′ examination, and the subclinical visual impairment was found in 2 of them with unobstructed clinical performance. All patients underwent high-dose Methylprednisolone therapy. The clinical symptoms of 16 patients who were treated with Gamma globulin were relieved in the acute phase.Seven patients had recurrence during the follow-up period, with the recurrence rate of 25.0%.Relapsed patients re-treated with high-dose Methylprednisolone therapy combined with Gamma globulin, clinical symptoms could be alleviated. Conclusion:The main clinical phenotype of children with central nervous system demyelinating diseases and seropositivity to MOG is acute disseminated encephalomyelitis. The spinal cord lesions are mainly involving cervical and thoracic segments. The current treatments of this disease include glucocorticoid and Gamma globulin, which have significant effect, but the disease is easy to relapse. The re-use of glucocorticoid and Gamma globulin after relapse is still effective.

ABSTRACT

FLIP is a protein containing death domain.Human beings have three subtypes of c-FLIPL,c-FLIPs and c-FLIPR,which can inhibit the apoptosis of a variety of tumor cells.c-FLIPL plays a dual role in the apoptotic signal. The expression level of FLIP not only decides the opening and closing of the apoptosis pathway but also achieve the conversion of cells in the apeptotic signaling and proliferative signaling pathway. The regulation of FLIP's expression is a multi-layered,involving multiple signaling pathways.FLIP will probably become an attractive death receptor signaling target of therapy.

ABSTRACT

Objective To investigate the features of variation of sympathetic skin response(SSR)in depression patients and to clarify the correlation between SSR and the effect of antidepressant therapy in first episode depression. Methods The sample comprised 50 first visit patients, who fulfilled the ICD-10 criteria in a structured interview for depressive disorder. These 50 patients received antidepressant therapy for 6 weeks.Clinical response was defined as ≥50%reduction in total 17-item Hamilton depression rating scale(HAMD).Remission was defined as HAMD score ≤7 at week 6.SSR waveforms were classified into three patterns based on the amplitudes of positive and negative components:P pattern,N pattern and M pattern.Another 50 healthy subjects were involved in this study as controls. Results (1)SSR could be obtained in all normal subjects but couldn't be recorded in 5 of 50 patients whose HAMD scores>28.SSR waveforul patterns were analyzed in these 45 patients P pattern in 7 cases,N pattern 21,M pattern 17.(2)Depression patients had longer latency and lower amplitude than the controls(P<0.01).P pattern had shorter latency and higher amplitude than N pattern. There were no difference in age, course and HAMD scores among the patients with 3 different SSR patterns.(3)Nine of 21 patients of N pattern treated with fluoxetine were remitters.6 patients were clinical responders. These 15 patients had shorter latency and higher amplitude after treatment than that at the initial stage, but there were still differences in latency and amplitude compared with the controls. Conclusion There were significant damages on sympathetic function of depression patients.N pattern was the most SSR pattern among 45 patients recorded SSR.P pattern had shorter latency and higher amplitude than N pattern.SSR is a sensitive and feasible measure inder for neurocardiac electrophysiologic autonomic regulation examination, which could be used to measure clinical therapeutic efficacy for antideppressant in depression patients.

ABSTRACT

Objective To understand the mycoplasma pneumoniae(MP) infection degree in community children by testing specific IgM antibodies against MP in different age bracket group in Shanghai Meilong area. Methods Using random sampling method, blood specimens of 1 817 children from kindergartens and primary or junior high schools in Meilong area were obtained. Children were from 2 to 15 years old, 969 males, 848 females. The specimens were tested for IgM antibodies against MP using with gelatin particle agglutination test. The data were statistically analyzed using with ?2 test. Results Five hundred and fifty-nine (30.7%) IgM antibodies against MP were positive from 1 817 blood specimens. The positive percentages were 27.34% and 34.66% for males and females, which had significant difference(?2=11.383 P=0.001). The higher percentage was detected from kindergarten children than primary and junior high school children(P=0). The positive percentages of anti-mycoplasma IgM had no significant differences between different kindergartens and primary schools(P=0.526,0.232). On the contrary, between different junior high schools, there were siginificant differences (?2=9.825 P=0.002). Conclusions MP is an important pathogenic mycoplasma cause for respiratory tract infections in Meilong area. It is relative to childhood asthma. The prevention and cure of MP infection for children shall be paid more attention.

ABSTRACT

The corn germ protein tested was a by-product from, corn starch plant and it contained more lysine (average 5.7%) and a relatively ideal proportion of eight essential amino acids similar to the 1973 FAO/WHO provisional amino acid pattern. A 5-week rat growth experiment was conducted to evaluate the nutritive value of corn germ protein by using body weight gain, nitrogen balance and apparent net protein utilization as assessment criteria. The feed of the experimental group was made of wheat protein supplemented with 17.3% germ protein and that of the control with 2.4% wheat glu- ten to made the final protein level of both feeds equivalent to 10%.These results showed that the body weight gain of rat in the experimental group was higher than that of the control group regardless of female or male, the body weight gain per 100g feed consumed in two groups of female was 19.0g and 12.2g and that of male was 17.6g and 10.4g respectively (p
